Calhoun Home Health is a Home Health agency located in the heart of Calhoun County that provides care to homebound patients who have a medical need that requires nursing and/or rehabilitation services. Our skilled team of nurses and therapists provide in-home services to individuals and families who need help managing their healthcare needs. At Calhoun Home Health, our goal is to return our patients to their normal pattern of living as soon as possible. We strive to maintain our patients in their home as long as possible by utilizing a variety of restorative methods and procedures designed to help our patients regain their confidence and independence.

To provide a broad base of services based on our patients' needs, involving patients in their own healthcare, establishing & implementing a plan of care, and evaluating the services provided.

To maintain patients in their homes as long as possible by providing nursing care and related health services to prevent unnecessary hospitalizations.

To assist patients in using all restorative methods, tools, and procedures to return to their normal pattern of living as soon as possible.

To develop a relationship with referral sources to effectively meet the home care needs of patients.

SERVICES

Calhoun Home Health offers a broad range of services to our patients. Whether recovering from an illness, surgery, injury, or has a long-term condition, our nursing staff and therapists will provide our services in the comfort of your own home.

Our services include, but are not limited to:

IV Therapy: Intravenous therapy at home is a way to receive IV medications that your doctor has ordered without having to go to a hospital to receive them. We offer this service as a safe and effective alternative.

Chronic Disease Management: We help patients, and their families, manage their disease process through care, education, and support.

Wound Care: we work in coordination with your doctor or outpatient wound care clinic of your choice to assist in the treatment and healing of acute and long term wounds with the use of dressings and/or wound vacs.

Therapy Services: Our physical, occupational, and speech therapists will come to the home to evaluate you or your loved one’s physical abilities and coordinate a complete therapeutic care plan with the primary care physician and/or specialist to help them return to their prior level of function.

Medication Management: Our team can help patients and family learn to effectively manage medications by providing education on medicine indications, educating on the use of pill planners and other strategies to ensure that your patient stays compliant with their medications.
